14U, 94, 93, 92, 91, & 90/89 (no High School Seniors at tryouts)
will go through the District Selects process.
Along with the
1) Registration form,
you must fill out, sign, & send the
2) Player Code of Conduct,
3) USA Consent to Treat,
4) USA Waiver of Liability forms,
5 )proof of 06/07 USA Hockey player registration
(copy of IMR form or USA Players card),
6) payment of registration fees.

Players failing to follow these steps fully will have their forms returned.
